Vermedx® Diabetes Information System
The Vermedx Program requires no provider or patient data entry. The system is driven by blood test reports from participating laboratories. Each time an enrolled patient has a diabetes-specific lab test, the results are sent electronically to VDIS within 24 hours.
The VDIS algorithmic engine immediately generates a Flow Sheet containing A1C, serum lipid, M:C and creatinine values for both the current test and those preceding it. If test result values are outside of the target range, the system will highlight these outliers on the Flow Sheet, and generate an Alert Letter, which is mailed to the patient.
If a patient is overdue in setting up an appointment, the VDIS system sends both Provider and Patient a Reminder.
The most powerful Vermedx tool for managing a physician’s total diabetic population is the quarterly Population Report. This document provides a handy format for analyzing progress against improvement goals for both individuals and the entire population, and compares them to nationally established benchmarks.
