For Providers
Overview
The Vermedx® system was created by a team of diabetes disease management experts and primary care physicians to assist clinicians in improving health care outcomes for diabetic patients.
Although traditional diagnostic and treatment guidelines are broadly known, new evidence-based best practices are continually evolving to combat a disease that affects 8 percent of the American population. The Vermedx® system puts these tools at your disposal in a unique timely and user-friendly format that also enhances patient compliance with appointments and follow-up treatment.

Benefits to Patients and Provider Practices
The subject of a five year clinical trial sponsored by the National Institutes of Health (NIH), the Vermedx® system’s combination of readily accessible treatment support information provides multi-faceted benefits including:
- A structured reporting system that instantly alerts clinicians and patients to blood testing results and prompts follow-up actions.
- Monthly reports that monitor the health improvement of your diabetic patient population against established benchmarks.
- A process that requires no financial investment and only a few hours of office staff time to learn how to use the program.
How It Works
There are three central components to the program::
- A Registry that contains relevant health data for diabetic patients.
- A participating testing laboratory that is electronically connected to the Vermedx® system.
- The Vermedx® automated decision support and alert system.
Each time that a clinician prescribes a blood test for a patient, before or following a scheduled appointment, the testing laboratory electronically transmits the test results to the system.
The Vermedx® system’s patented algorithmic technology then automatically generates the following documents:
- A Flow Sheet detailing test results, implications, and evidence-based best practices recommendations. These sheets are faxed or transmitted by secure email connection to the clinician’s office.
- A Patient Reminder Letter is sent to patients who are overdue for a follow-up appointment, based upon test results.
- A Provider Reminder is faxed or emailed to the clinician’s office if the patient is overdue for a follow-up appointment.
- A Patient Alert Letter is sent to patients whose recent test results require prompt action. The letter directs the patient to call their provider to arrange care.
Each month, a Population Report is sent to each clinician that summarizes the health improvement progress of every diabetic patient compared to established benchmarks.
To notify Vermedx® of patient status changes or of a new patient to be added to VDIS, use this convenient Patient Status Change fax form.
Investment
The only requirements for practices that already have fax or Internet capability are developing connectivity with the practice's clinical laboratory, and a few hours of staff time to setup the Vermedx® program.
All aspects of the program have been certified as HIPAA compliant. Our experience indicates that more than 95 percent of diabetic patients agree to participate in the Vermedx® program.
We are proud to say that we have received numerous letters and other expressions of gratitude for the difference that our program has made in patients’ and providers' lives.
